Dear Student:

Again this year, we will offer up to three of our $1,000 memorial scholarships to past recipients and alternates of the PTSC Scholarship Program.  To qualify for the renewable scholarship, applicants must:

• Provide a current college transcript
• Maintain a 3.0 GPA
• Be a full-time student (averaging 12 hours per semester) in an undergraduate program
• Write a short, concise essay (300-500 typewritten words).  The cover page must list your
  name, address, major, estimated graduation date, and current phone number. 
  Essay must cover the following:

1.   How did receipt of the PTSC Memorial Scholarship make a difference in your
college experience to date?
2.   How have your views of the value of education changed since you began your
college experience?
3.   Summarize any ways in which your education to date has changed your views   
of personal identity, family values, American citizenship, and caused
any changes in your overall world view.

      Renewable scholarship applicants will be judged on the following criteria:
65%           personal and leadership qualities
35%           academic accomplishment (minimum of 3.00 grade point average)

      Applications must be turned in by 5:00 p.m. on February 26, 2010 to either Physical Therapy Specialists Clinic location in West Plains or Mountain Grove.  The $1,000 scholarship will be awarded in West Plains at an awards banquet in April, 2010.  If you have any questions please call me at 1-800-695-5899, or e-mail me at cjones@ptsconline.com.
