A. Provide your actual GPA (please convert to 4.0 scale)
Category II
Personal and leadership qualifications:
A. Identify your top five honors, leadership positions, or awards attained in high school and briefly explain why you consider these your most significant accomplishments.
B. List other honors, activities, club memberships, etc.
C. Indicate any community service (defined as volunteer work done in the community, excluding religious or Church related activities) and the number of volunteer hours contributed per week.
D. List Church or religious organization activities.
E. Indicate any work experience.
Category III
List your future plans and specific college interests.
