Applications for the $1,000 college scholarship must be typed utilizing the attached format or they will be disqualified.  Handwritten applications will not be accepted. 
GPA, ACT and/or SAT scores must be verified by your school counselor.

Applicant, please utilize the following format as much as possible when preparing your typewritten application.  You may attach other information to the application you feel the scholarship board needs to take into consideration. 

Category I

Academic:

A.        Provide your actual GPA (please convert to 4.0 scale)

• minimum score of 3.0 accepted
• Provide your ACT Composite Score
• minimum score of 23 accepted
• Provide your SAT score (if applicable)
• minimum score of 1100

Category II

Personal and leadership qualifications:

A.        Identify your top five honors, leadership positions, or awards attained in high school and briefly explain why you consider these your most significant accomplishments.
B.         List other honors, activities, club memberships, etc.
C.        Indicate any community service (defined as volunteer work done in the community, excluding religious or Church related activities) and the number of volunteer hours contributed per week.
D.        List Church or religious organization activities.
E.         Indicate any work experience.

Category III

List your future plans and specific college interests.
